Agreement to Terms
By accessing our website, booking a class, purchasing a class pack, membership, or any service from Formacore Pilates LLC (“Formacore,” “we,” “us,” or “our”), you acknowledge that you have read, understood, and agree to be bound by these Terms & Conditions, along with our Liability Waiver and Privacy Policy.
These Terms apply to all clients, whether booking online, through our booking platform, or in-studio. If you do not agree to these Terms, you should not purchase, book, or participate in any Formacore services.
Formacore reserves the right to update or modify these Terms at any time. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.
General Terms and Conditions
Unlimited monthly packages include up to 1 class per day. Additional classes on the same day will be charged to the credit card on file at $20 per class.
Recurring memberships automatically renew after completion of the initial commitment. Email us at team@formacorestudio.com to change or cancel your recurring membership, after the initial commitment has been met and with at least 10 days before your next scheduled payment. In the event of cancellation before the initial commitment has been met, you will pay the difference between the full priced package and the discounted contract package for the number of completed payments made under the contract at time of early cancellation.
You can freeze your monthly recurring package for suspensions over 10 days. Please email team@formacorestudio.com at least 7 days before the date you want to have your package suspended.

Formacore accepts Visa, Mastercard, American Express, or debit.
All sales are final. Classes are non-refundable. However, your package can be transferred to another client.
It is required that clients wear grip socks during class. These are available in the studio for purchase.
The studio opens 15 minutes before class. First time clients must arrive no later than 10 minutes before class for a safety orientation on the XFormer.
Machines may be released to clients on standby at the start of class. If you are late and your reserved space is given to a drop-in client, we will not charge you and you will keep your session purchased through Formacore, to use another time. If you are on a Unlimited Package, please email us at team@formacore.com so that you will not be charged a no-show fee.
Class Cancellations
Need to cancel a session? Don’t sweat it! Due to our small class size we have a 6-hour cancellation policy. This means you can cancel your class up to 6hours before the class start time and the class will be re-credited to your account for future use.
Classes cancelled after the 6 hour window, will be considered late cancellations. Session clients will lose that session, and members will be charged a $15 fee for late cancellations and $25 for no shows. The charge typically occurs 7-14 days after the cancellation/no show.
The intro offers; First Class and Intro offers are valid one per client only.
For emergencies and other last minute requests, please email team@formacorestudio.com. We will evaluate requests on a case by case basis and try our best to accommodate emergency cancellations. However, we reserve the right to uphold our late cancellation policy.
Waitlist
If the class you want to attend is full, join the waitlist. To join the waitlist you must have a class credit in your account.
If a spot opens up, you will be notified by email. To receive these reminders, your profile must be set to accept notifications and reminders.
You can remove yourself from the waitlist up to 6 hours before the class starts. Once inside the 2-hour window before class, no further clients will be pulled from the waitlist. Please be sure to remove yourself from the waitlist if you no longer plan on joining the class — if you are pulled off the waitlist and don’t show up for class, you will lose that session, or be charged a $25 no show fee if you are a member.
Use of Image & Likeness
From time to time, Formacore may take photos or videos for marketing or promotional purposes. We will always notify clients in advance when photography or recording is taking place.
There are no permanent cameras in the studio, and no photos or videos are taken during class without notice.
By participating in a class or event where photography or recording has been announced, you grant Formacore permission to use your image, likeness, and voice in captured content for marketing and promotional use across our platforms. No compensation will be provided.
If you prefer not to be photographed or recorded, please let a team member know prior to class and we will fully accommodate you.
Communications
By booking or purchasing with Formacore, you agree that we may contact you via email, text message, or phone for purposes including, but not limited to: Class confirmations or schedule changes, late arrival or no-show follow-ups, emergency notifications, checking in if you have not arrived for a scheduled class.
You may opt out of non-essential marketing communications at any time, but transactional and safety-related messages may still be sent.
Mobile Program Description
By opting in, you agree to receive recurring automated text messages from Formacore ("we," "us," "our") at the mobile number you provided. Messages may include [appointment reminders, order updates, account alerts, promotional offers, customer support follow-ups, etc.. Consent to receive these messages is not a condition of any purchase.
Message Frequency
You may receive up to 4 messages per month. Message frequency may vary depending on your activity and interactions with us.
Message and Data Rates
Message and data rates may apply. Charges are billed by and payable to your mobile service provider. Please contact your wireless carrier for details about your messaging plan.
How to Opt Out and Get Help
You can unsubscribe the SMS service at any time by replying STOP to any message we send you. After you send STOP, you will receive a one-time confirmation message, and no further messages will be sent. If you want to rejoin, you can sign up again using the original opt-in method or reply START. For help or more information at any time, reply HELP to any of our messages. Carrier Liability Disclaimer
Carriers are not liable for any delayed or undelivered messages.

Third-Party Services
We may use third-party services, such as payment processors and scheduling tools. We are not responsible for their security, performance, or privacy practices, and your use of such services is at your own risk.
Code of Conduct
Clients agree to conduct themselves respectfully toward staff, coaches, and other participants. Formacore reserves the right to refuse service or membership access to anyone whose conduct is deemed inappropriate, unsafe, or detrimental to the community.
Changes to Terms
Formacore reserves the right to modify, update, or amend these Terms at any time. Notice of significant changes will be communicated on our website or in studio communications.
Governing Law & Jurisdiction
These Terms shall be governed by the laws of the State of Iowa, without regard to its conflict of law principles. Any disputes shall be resolved exclusively in the state or federal courts of Johnston County, Iowa, and you consent to the jurisdiction of such courts.
